We may collect the following personal information from you when you interact with our website, phone services, or intake systems: Name, Relationship to the person seeking care, Age, Zip code, Phone number, Email address, Healthcare and home care needs, Mobility needs, Status regarding enrollment in Medicare or Medicaid. For quality assurance and automated onboarding, we also collect inbound call records and call transcripts from callers.
Information We Do Not Collect
We explicitly do not request or collect the following information: Social Security numbers, Medicare identification or account numbers, Medicaid identification or account numbers, Any healthcare identification or account numbers.
How We Use Your Information
We use the information we collect for the following purposes:

Storing Personal Information
We store and process your personal information securely using the following platforms: HighLevel, LLC, Google Cloud Platform (Vertex AI/Agent Platform), LeadsHook Pty Ltd., and Private Google Drives.
We implement appropriate security measures, such as access controls, to ensure your data is protected. Please note: We do not use manual database encryption. Only authorized personnel have access to your information.
All call transcripts processed by our artificial intelligence infrastructure are handled statelessly via an enterprise Business Associate Agreement (BAA). Transcripts are processed in real-time, never permanently stored by the AI sub-processor, and are never utilized to train public machine learning models.
Data Sovereignty and Regional Processing
We strictly enforce data sovereignty. All patient data, call transcripts, and personally identifiable health information (PHI) are processed and housed exclusively on secure cloud infrastructure located within the United States (us-central1). No clinical or contact data is transferred or routed internationally.
Sharing Personal Information
Your personal information may be shared with the members of the Kansas PACE consortium, which exclusively include: Midland Care Connection PACE, Bluestem Communities PACE, Ascension Living PACE. We do not sell your personal information to third parties. However, we may share it with our consortium members for the purpose of providing and improving PACE-related services. Please note: We do not share any personal health information with third-party advertising or analytics providers.
Sharing Cookie Data and IP Addresses
We may share non-personal data, such as cookie data and IP addresses, with third-party service providers for marketing, analytics, and advertising purposes. These third parties include: NextRoll, Inc. (AdRoll), Meta Platforms, Inc., Microsoft Advertising, HighLevel, LLC, Kick, LLC, Google LLC (including Google Analytics), and LeadsHook Pty Ltd.
These third parties use this data to provide targeted advertisements, measure campaign performance, and improve website functionality.
